We have provided this (optional) online feedback form for you to use to send us your feedback on the Simple Format Reporting Standards. 

We encourage you to use it.  Providing your feedback in this way will make it easier for us to analyse your feedback.

Use this form to add your responses to the questions below. 

The form contains all the questions included in the Request for Information.

You may choose to answer all questions in Section B or respond only to those questions of most interest to you.
